What is diplopia?
Diplopia (double vision) is the perception of the image of an object as if that object is more than one. Patients may experience diplopia in one or both eyes. Binocular diplopia is the perception of the image of an object as if that object is more than one when both eyes are open.
It is usually caused by a problem with the eye muscles and the nerves that nourish the eye muscles.
The eyes cannot look at the same point in coordination and double vision occurs. Under normal circumstances, the visual focus should be parallel.
People with binocular diplopia can see everything easily with one eye, but when both eyes are opened, they see double. The most common cause of this condition is neurological diseases.
There may also be double vision, called monocular diplopia, in one eye. In this case, the cause is usually the eye’s inability to focus on a single point. This condition is most often caused by an eye disorder called astigmatism.
Symptoms and Treatment of Diplopia
Diplopia can occur due to other serious problems besides the eye. The complaint of diplopia should be taken seriously and a doctor should be consulted immediately.
Additionally to double vision, people may experience other clinical symptoms such as headache, dizziness, inconsistency in eye movements, droopy eyelids, facial paralysis. These conditions may vary depending on the root cause of the problem.
Regardless of whether you experience neurological symptoms other than diplopia problems, you should see a doctor for eye and neurological examinations.
There is no direct cure for diplopia. Because in fact, this is not the disease itself, but the symptoms of other diseases. Therefore, in order to correct this situation, the underlying disease must be found firstly.
After the diagnosis of the disease with the necessary examinations, the treatment of the disease will begin. When the disease causing the problem is treated, the diplopia disappears anyway. It is possible to get rid of the diplopia problem within the scope of the service we provide with specialist doctors in the field.
Diplopia is permanent when the disease rarely causes permanent nerve damage. In this case, we can save our patients from their complaints by using prismatic glasses.
What are the Causes of Double Vision (Diplopia)?
Diplopia can be a symptom of the important diseases we have mentioned, or it can be temporary. A detailed neuro-ophthalmological examination is absolutely necessary to fully understand the cause.
If double vision is observed with one eye, this indicates that it is due to eye diseases. If the diplopia disappears when one eye is closed, this indicates that the underlying cause is neurological.
Can Double Vision Be Caused by Neurological Problems?
The first question that comes to mind when faced with diplopia is whether it is caused by a neurological disease. Neurological problems may be considered when diplopia occurs consistently. It is likely to be caused by neurological problems, especially when the condition becomes permanent or recurs frequently.
In this case, it would be more beneficial to perform detailed eye and nervous system examinations in people. We, as Retina Academy, conduct the necessary tests for people with double vision problems.
Within the scope of these tests, it can be understood whether the disease is of neurological origin and you will be directed to the relevant unit for further monitoring methods.
For the treatment of the disease, it is necessary to find the problem that caused it. Otherwise, life-threatening diseases can be missed. Diseases that may result in permanent vision loss or even death may go unnoticed.
Which Neurological Disorders Cause Double Vision?
In general, neurological problems are emphasized in cases such as double vision. Due to the damage to the bone wall that makes up the eyeball, the nerves that coordinate the movement of the eyeball become paralyzed and cause sudden strabismus.
This is one of the common neurological causes. The most common diseases that cause this are ballooning of the brain vessels (aneurysm) and intracranial tumors. Apart from these, head trauma is one of the common causes of diplopia.
Diplopia in the eye muscles can also occur due to thyroid diseases, some muscle and nervous system diseases, increased intracranial pressure, blockage of the brain sinus cavity or blood vessels, and similar reasons.
These are the most common causes of binocular double vision (ie diplopia occuring when both eyes are open). There are also different causes of diplopia for one eye. Retinal diseases and conditions that cause deterioration of the refractive structure of the cornea are the leading ones. Deterioration or damage to the lens structure is also among the causes of diplopia.
At Which Stage Should Those Who Have Double Vision (Diplopia) Consult a Doctor?
The problem of diplopia must be consulted a doctor. Even if it recovered in a short time, evaluation is required. Another important question in the minds of people with diplopia is of which department or doctor to go to for this problem.
Since the problem may be caused by eye diseases or neurological problems, it is an important question to which department it will be applied.
It should not be forgotten that when you have a double vision problem, you should apply to any health institution you can reach as soon as possible to determine whether you have multiple vision problems when you look with one or both eyes.
Ophthalmology or neurology department usually deals with diplopia problems. If you’re having this problem while looking with one eye, it’s likely the eye is causing the problem. If you’re having this problem while looking with both eyes, the problem is usually caused by the nervous system.
What Kinds of Examinations Are Performed in a Patient Presenting with the Complaint of Double Vision (Diplopia)?
Patients with diplopia should undergo a detailed eye examination. Vision level, eye movements and pupils are evaluated primarily. Then the optic nerve and retina are examined. If necessary, MRI and CT examination or cerebral angiography may be requested.
If it is thought to be of neurological origin, MRI should be requested definitely. Thyroid and antibody tests can also be done in relation to goiter disease. EMG examination may be required to examine the eye muscles.
As a result of all these tests and examinations, it is determined what causes the diplopia problem. Depending on the cause, the most appropriate treatment is preferred and the disease is treated. Thus, the problem that causes double vision is completely eliminated.
Sometimes, prismatic glasses can be used to eliminate double vision until the imbalance in eye movements is corrected. Thanks to these glasses, it is aimed that you can do your daily works more comfortably.
Causes of Temporary Diplopia
There are two different types of diplopia. They are divided into temporary and permanent. Temporary diplopia usually resolves on its own, depending on the cause.
The most common cause of this is the damage to the nerve that nourishes the eye muscles which is seen in patients with hypertension or diabetes. Sometimes, working too closely or using the wrong glasses can cause double vision complaints. When you are too tired, temporary double vision may occur.
